Death Certificate Checklist & Info Military

Active Duty – See military affairs coordinator for death Certificates (Arlington will provide 1 outer burial liner (oversize as well) (Sandhills will provide 1 outer burial container) *Subject to change per Arlington & Sandhills Veteran Affairs*

Retired Military - Officer can get up to 5 Free Death Certificates (Check with Mortuary Affairs Coordinator regarding Retired Military outer burial liner/container) *Subject to change*

Sandhills – Will need A) DD214, B) 2 Proofs of Residency (can Not be an utility bill) & C) Marriage Certificate, if applies). Please call 919-664-1175 to verify fees and outer burial containers, at need. Family will not be able to go to burial site, day of service, until after 4pm the day of the service. Flowers will not be able to stay after service but can be taken back after 4pm to be placed on the burial site. Opening & Closing fee is currently $800 for any non-military dependent, the Family is responsible for this. *Subject to change per Sandhills Veteran Affairs*

If decedent has a spouse already at Sandhills, we will need their name. If there is no spouse already at Sandhills, we will need next of kin’s full name, DOB, social security number, mailing address, and their relationship to decedent.
